Buying a top on its own has two advantages over a full set: you can mix sizes across pieces (say, one size up on top, true to size on the bottom), and you gain wardrobe variety — rotating several tops with the same bottom is both more economical and more practical.

Collar choice mostly comes down to specialty and personal taste: a V-neck feels more open and gives freedom of movement around the neck (useful in the OR or ICU), while a round neck reads more classic and covered — often preferred for outpatient clinics and private practice. Pocket layout matters too — a chest pocket for pens and ID, a kangaroo or side pocket for a phone and small tools.
